STATE OF WASHINGTON SCHOOL IMMUNIZATION REQUIREMENTS
Washington state law requires certain immunizations against childhood diseases for all students, regardless of age, who attend a public or private school or a licensed day care center.
On or before the first day of attendance, the school must receive proof of the child’s:

• full immunizations,
• initiation of schedule of immunizations, or
• certificate of exemption, current with the 2011-2012 state requirements.

A completed and signed Certificate of Immunization Status (CIS) (English) (Russian) (Spanish) is required to be on file at the school, preschool, or child care facility.
The Washington State Department of Health offers MyIR for secure, online management of family immunization records. Once you register, you can access the records any time and even print your child’s Certificate of Immunization Status (PDF) for school entry. Register for MyIR.

*2011 Changes to School/Child Care Immunization Exemptions
Effective July 22, 2011, Washington State Law requires a licensed health care provider to sign the Certificate of Exemption in order for a parent or guardian to exempt their child from school and child care immunization requirements. The signature verifies that the health care provider has provided the parent/guardian information about the benefits and risks of immunization. A signed letter from a health care provider stating the same information is also acceptable.
This law only applies to exemptions requested after the July 22, 2011 date. Parents/guardians who demonstrate membership in a church or religious group that does not allow a health care provider to provide medical treatment to a child only need to complete the Certificate of Exemption form, Box 2, in order to be in compliance.
